How To Remove Equipment from Campus
Summary: Use this step-by-step guide to temporarily remove inventorial equipment from campus for campus business, research, etc.
Completion of the Notification of the Removal of Equipment from the Campus Form allows the requesting department and Equipment Management to record where and how long the equipment will be off campus.

What the Requestor does: |

Ask in writing for permission to remove the equipment from campus. |
Fill in the Notification of the Removal of Equipment from the Campus Form:
- Provide the property number, description, serial number, and value of each piece of equipment.
- Provide the temporary location information, date of removal, estimated date of return, and reason for removal.
- Sign and date the form, and forward it to your Unit Head.

Approve the removal of the equipment from campus. |
Sign the completed Notification of the Removal of Equipment from the Campus Form and forward it to Equipment Management, 1156 High Street, Santa Cruz CA 95064 Phone: 831-459-2355, Fax: 831-459-3747. |

Record the new location of the equipment. |
Transfer the equipment location in the database to location 7916: "in the field." |

When the equipment is returned to campus: |

Notify Equipment Management that you have returned the equipment to campus. |
Fill in and submit the final ("returned") portion of the Notification of the Removal of Equipment from the Campus Form to Equipment Management. |

Record the correct location of the returned equipment. |
Upon receiving the "returned" portion of the Notification of the Removal of Equipment from the Campus Form, edit the database to reflect the correct location code.

All property of the UC Regents is automatically covered under the Self-Insured Property Program, outlined in BUS-28 Property Self-Insurance Program. This insurance is only available in excess to any other applicable insurance for any loss or damage to this property while it is in the Requestor's care and custody.
